Develop and implement individualized OT care plans focused on improving ADLs, IADLs, safety, participation, and environmental adaptations . Educate patients and families on therapeutic techniques , compensatory strategies, energy conservation, home exercise/engagement programs, and safe use of adaptive equipment. Collaborate with interdisciplinary teams to ensure continuity of care , effective communication, and safe discharge planning. Maintain accurate documentation of patient progress, functional outcomes, and clinical reasoning while adhering to professional OT standards and medical terminology. Stay current with best practices in occupational therapy , particularly in home health, geriatrics, functional rehabilitation, and aging‑in‑place strategies. Considering a move to Washington State?
