Requirements
- Current Florida Occupational Therapist (OT) license (required)
- Excellent communication, time management, and documentation skills

Nice To Haves
- Previous home health experience is strongly preferred but not required
Responsibilities
- Conduct Occupational Therapy Evaluations to establish individualized plans of care
- Evaluate patients’ functional abilities related to activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
- Develop and implement personalized treatment plans to improve independence, safety, and quality of life
- Provide therapeutic interventions addressing self-care, home management, cognition, and upper extremity function
- Educate patients and caregivers on adaptive techniques, energy conservation, home safety, and use of adaptive equipment
- Collaborate closely with physicians, nurses, physical therapists, speech therapists, and other care-team members to coordinate care
- Monitor patient progress, update care plans, and maintain thorough, compliant documentation
- Ensure compliance with all Medicare, CMS, and home health standards
