Requirements
- Master’s degree in Occupational Therapy
- Active Arizona OT license
- Current CPR/BLS certification
- Home health experience preferred
- Strong communication and organizational skills
- Reliable transportation and valid Arizona driver’s license
Responsibilities
- Evaluate patients’ functional abilities in their home environment
- Develop and implement individualized treatment plans
- Help patients improve daily living skills and independence
- Educate patients and families for long-term success
- Collaborate with nurses, PTs, and interdisciplinary team members
- Complete timely and accurate documentation
