About The Position

Occupational Therapy Assistant provides skilled Occupational Therapy service on an intermittent basis to clients in their homes under the supervision of the Occupational Therapist. Provides c lient care services which are delegated by- and under the direction of- the Occupational Therapist (OT). Assists in evaluation and developme n t of the rehabilitative plan of care and in period i c re-evaluations as required under the direction of the OT. Participates in the preparation of clinical and daily progress no t es which are reviewed by the OT and enters in EMR within 24 hours. Also prepares periodic summary reports. Assists the OT in providing instructions for the client and family and in demonstrating exercise programs. Guides and instructs the client in prescribed therapeutic self- c are methods and devices as well as creative activities that are directed toward improving independe n ce and physical and m ental functioning. Identifies and reports possible ineffective drug therapies or adverse reactions, significant side effects, drug allergies, and contra-indicated medications to the O T. Observes and records activities and findings in the clinical record; reports the client ' s reactions to treatment and any c h anges in the client ' s condition to the OT and the physician. C on s ults with O T regard i ng the outco m e of ho m e visits and repo r ts physical findings in writing. I n structs other health team personnel includ i ng ho m e health aides and family members, as appropriate, with respect to certain phases of occupational therapy so that they may work with the client. Attends rehabilitation department staff meetings and case conferences. Instructs client in care and use of wheelch a irs, braces, splints, and p r osthetic and orthotic devices. Participates with staff, c l ients, and physicians in discharge activities and assists the OT in completing the Occupational Ther a py Discharge Su mm ary. Confir ms , on a weekly basis, the scheduling of visits with the S upe r v isor / Director of Nursing in order to c o ord i nate A g en c y visits. Assumes responsibility for s e lf-development by continually striving to imp r ove his/her health care knowledge through education a l progra m s, attendance at wo r k shops and conferences, active participation in p r ofessional and related org a nizations and i nd i vidual research and reading. Assu m es other du t ies dee m ed app r opriate and necessary by the OT. Is knowledgeable of the Agency’s organization, functions, and services. Complies with Agency dress code policy. Maintains efficient flow of Agency work by reporting to work at assigned or scheduled items; follows procedures for notification of absence or time off requests. Maintains current TB testing (refer to Health Screening Policy).

  • Consults with OT regarding the outcome of home visits and reports physical findings in writing.
  • Instructs other health team personnel including home health aides and family members, as appropriate, with respect to certain phases of occupational therapy so that they may work with the client.
  • Attends rehabilitation department staff meetings and case conferences.
  • Instructs client in care and use of wheelchairs, braces, splints, and prosthetic and orthotic devices.
  • Participates with staff, clients, and physicians in discharge activities and assists the OT in completing the Occupational Therapy Discharge Summary.
  • Confirms, on a weekly basis, the scheduling of visits with the Supervisor / Director of Nursing in order to coordinate Agency visits.
  • Assumes responsibility for self-development by continually striving to improve his/her health care knowledge through educational programs, attendance at workshops and conferences, active participation in professional and related organizations and individual research and reading.
  • Assumes other duties deemed appropriate and necessary by the OT.
  • Is knowledgeable of the Agency’s organization, functions, and services.
  • Complies with Agency dress code policy.
