About The Position

We are seeking a dedicated and experienced Registered Nurse (RN) to provide 1-on-1 intensive home health care for a 1-year-old patient in the Ann Arbor area. This role requires a nurse with strong pediatric assessment skills and a background in managing complex neurological and gastrointestinal needs. We have immediate needs for private duty nurses (RNs or LPNs) in the West Bloomfield Township area to provide home care to pediatric patients. Our pediatric population ranges in ages from birth to 21 years of age. Founded in 2009 and headquartered in Farmington Hills, Michigan, Centria Healthcare is a leading provider of home-based catastrophic injury and rehabilitation care, private duty nursing, and supportive living services across the state of Michigan. Centria coordinates among multiple disciplines for its rehabilitation services including physical therapy and occupational therapy. Transcending beyond the role of traditional home healthcare providers, Centria has established itself as a market leader through its diverse expertise spanning from the rehabilitation of those who have experienced a Traumatic Brain Injury or Spinal Cord Injury to children dependent on technology. Requirements

  • Active RN or LPN license in the state of Michigan
  • Strong communication skills and a passion for patient-centered care

Nice To Haves

  • Experience in home health, private duty, or long-term care preferred
  • Pediatric or adult care experience a plus (trach/vent knowledge helpful but not required)

Responsibilities

  • Provide skilled nursing care (trach/vent care, medication administration, catheter care, etc.)
  • Monitor and document vitals, symptoms, and changes in condition
  • Assist with ADLs such as repositioning, hygiene, mobility, etc.
  • Support patient well-being with compassionate and attentive care
  • Collaborate with families, caregivers, and the healthcare team
