About The Position

KORT in New Albany, IN is seeking a PRN Home Health nurse case manager. KORT is Kentucky/Indiana's premier provider of outpatient physical and occupational therapy specializing in orthopedics, sports and industrial medicine. Since 1987, KORT has provided clinical and customer service excellence to patients in our clinics, at employer sites, and with high school, college, and professional sports. In addition, KORT Home Health also contracts their home health services in Kentucky including: post-operative physical therapy; fall prevention; vestibular rehabilitation; wound care; and more. Compensation starting: $50.00/visit plus mileage reimbursement. Open positions: 1

Requirements

  • Be a registered nurse (R.N.) with a current Indiana license.
  • Minimum of one year of nursing experience as a R.N.
  • Current automobile liability insurance and current unrestricted drivers license.
  • CPR certification is mandatory immediately after employment

Responsibilities

  • Deliver professional nursing care for client in accordance with the established plan of care and agency policies and procedures.
  • Provide care consistent with all relevant professional standards and state and federal requirements.
  • Demonstrate proficient skills using Oasis to admit, transfer, re-certify, and discharge home health patients in regards to physician orders.
  • Coordinate and supervise LVN' s and HHA's in the delivery of patient care.
  • Maintain compliance with agency policy and procedures.
  • Follow Medicare and Medicaid regulations.
  • Perform the initial home care patient visit (if indicated) and re-evaluate the patient's needs and progress on a regular basis.
  • Initiate the plan of care (if indicated) under doctor's orders.
  • Perform admission, transfer, re-certification, resumption of care, and discharge Oasis for the home care patient.
  • Observe, assess, and document symptoms.
  • Monitor reactions and patient progress.
  • Educate patients and caregivers on disease process, medications, plan of care, and individualized treatment plans.
  • Educate patients and caregiver on techniques for in home health care.
  • Coordinate patient services.
  • Supervise LVNs and HHA's.
  • Notify the physician and other personnel (Clinical Supervisor, PT, Case Manager) of change in the patient's condition.
  • Perform skill outline in the agency's approved policy and procedure manual.
  • Discharge the patient from skilled nursing services when the discharge criteria have been met.
  • Case conference clinicians providing care to ensure coordination of care.
  • Update clinical records according to policy and procedures.
  • Update knowledge and skills by attending in-service programs, continuing education programs, seminars, and self-study programs annually.
  • Provide onsite supervision of LVN/HHA.
  • Adhere to Medicaid and Medicare regulations.
