About The Position

Puget Sound Home Health & Hospice is seeking an experienced Part-time MSW or LICSW to join our home health team in King County (Kent, Burien, Renton, etc.) . Requirements

A graduate of an accredited master’s program in social work
Licensed MSW or LICSW in Washington State
Minimum of one year’s experience in health care
Possesses and maintains current BLS Certification
A licensed driver with an insured automobile

Nice To Haves

Experience with home health or hospice preferred

Responsibilities

Assess the psychosocial status of patients related to patient’s illness
Develop and provide multiple community and governmental resources to support a patient in their home environment
Provides assessment in patient’s residence and carries out social evaluations and plan interventions
Participate in the development of the individualized plan of care
Assist physician in understanding significant social and emotional factors
Participate in quality assessment performance improvement team
Serve as liaison between patients and community agencies
Maintain and develop contracts with public and private agencies
