Requirements
- Master’s Degree in Social Work (required)
- Minimum one year MSW experience within the last three years
- Flexibility to meet patient scheduling needs, including new admissions
Nice To Haves
- Home health or hospice experience preferred
- Experience with geriatric population preferred
Responsibilities
- Develop and implement individualized MSW plans of care in collaboration with physicians and the interdisciplinary team
- Assess psychosocial needs and determine eligibility for home health services
- Provide counseling, resource coordination, and crisis intervention
- Re-evaluate and adjust care plans as patient needs change
- Support patients and families navigating complex healthcare decisions
