About The Position

Puget Sound Home Health is seeking several experienced and caring full-time or part-time Licensed Practical Nurses (LPN) for our Home Health division in the Thurston County, Washington area starting in January! Never worked in Home Health before? Come on a ride-along with a seasoned Home Health nurse and learn more! A ccountability P assion for Learning L ove One Another I ntelligent Risk Taking C elebration O wnership Essential Duties Provide one-on-one skilled nursing care to patients in their homes (e.g., wound care, catheter care, medication administration, symptom management, etc.) Provide skilled observation, assessment, and evaluation Provide patient and family/caregiver support and education Practice cohesive and collaborative care with the healthcare providers
Requirements
- Current WA Licensed Practical Nurse (LPN), or before first day of work
- Ability to work independently and demonstrate critical thinking
- Current BLS certification from AHA
- Be a licensed driver with an insured automobile in good working order
Nice To Haves
- Prior Home Health and/or Hospice experience preferred
Responsibilities
- Provide one-on-one skilled nursing care to patients in their homes (e.g., wound care, catheter care, medication administration, symptom management, etc.)
- Provide skilled observation, assessment, and evaluation
- Provide patient and family/caregiver support and education
- Practice cohesive and collaborative care with the healthcare providers
