Requirements
- Active Washington State LPN license (or in place before first day of work)
- Strong critical thinking and ability to work independently
- Current BLS certification (AHA)
- Valid driver’s license with insured, reliable vehicle
Nice To Haves
- Home Health experience preferred but not required
Responsibilities
- Provide one-on-one skilled nursing care in patient homes, including: Wound care, Catheter care, Medication administration, Symptom management
- Perform skilled observation, assessment, and evaluation
- Educate and support patients and families/caregivers
- Collaborate with interdisciplinary healthcare teams
- Maintain accurate clinical documentation
