The role is for a motivated LPN with experience in Home Health Skilled Nursing Visits to join the team in Colorado Springs. In this position, the LPN will visit clients in their homes to assess and manage their needs and provide education.

Requirements

  • Valid LPN license in the state of Colorado
  • Minimum of 1-2 years of nursing experience, preferably in home health or a similar setting
  • Excellent clinical assessment and critical thinking skills
  • Proficiency in computer usage and documentation
  • Strong communication and interpersonal skills
  • Compassionate and patient-centered approach to care

Responsibilities

  • Provide skilled nursing care to patients in their homes
  • Work independently to assess patient needs, develop care plans, and implement appropriate interventions
  • Maintain accurate and thorough documentation of patient assessments, interventions, and outcomes
  • Communicate effectively with patients, families, team members, and other healthcare professionals to ensure coordinated care
  • Adhere to all regulatory requirements and company policies and procedures

  • Sign-on Bonus: Full Time - $2,000.00. Paid at 90 days of employment. Must have worked an average of 30 hours a week in the prior 90 days.
  • Sign-on Bonus: Part Time - $1,000.00. Paid at 90 days of employment. Must have worked an average of 20 hours a week in the prior 90 days.
