About The Position

In this role, you'll provide skilled nursing care to patients in their homes on an intermittent basis. What’s in it for you? Requirements

  • Must be a Licensed Practical Nurse with a current license to practice nursing in the Commonwealth of Virginia
  • Graduate of an accredited nursing school.
  • Must have a valid driver's license and a reliable car available for use during work hours.

Nice To Haves

  • At least one year of home health experience preferred

Responsibilities

  • Support the Registered Nurse in delivering each patient’s plan of care
  • Assist with specialized procedures under the RN’s direction and the physician’s orders
  • Contribute to planning, implementing, and regularly evaluating patient care plans
  • Recognize how social and economic challenges impact patients, and provide emotional and physical support for them and their families
  • Help educate patients and families on proper self-care techniques
  • Actively participate in team and staff meetings to promote coordinated care
  • Document clinical and progress notes after each patient visit
  • Monitor and report any symptoms, reactions, or changes in patient condition to the appropriate team member
  • Participate in a rotating weekend on-call schedule
  • Perform other duties as needed
