Facility: BayCare HomeCare Location: Bradenton, FL Status: Per Visit (PRN) Shift Hours: 8:00 AM - 5:00 PM Weekend Work: Occasional On Call: Yes Home visits in assigned service area: Manatee County The Per Visit Home Health LPN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include: Delivery of patient care to assigned patients, provides care for clients as ordered including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse Identifies, implements, and maintains appropriate teaching plans for patient and caregivers under direction of a Registered Nurse Follows plan of care and collaborates with Registered Nurse regarding patient response to treatment and change in status Utilizes nursing knowledge in the administration of medication and evaluation of effectiveness when applicable. Requirements
- BLS (Basic Life Support)
- LPN (Registered Nurse) License
- Driver's License State of Florida (Florida driver's license is required within 30 days of hire)
- Associate's or Diploma Nursing
- 1 year Licensed Practical Nurse

Responsibilities
