Requirements

• Current LPN license in Iowa.
• Experience in home health or a related healthcare setting is preferred.
• Strong organizational, communication, and problem-solving skills.
• Ability to work independently and deliver patient-centered care.
• Valid driver's license and reliable transportation for patient visits.

Responsibilities

• Conduct home visits to assess patient conditions, record vital signs, and document care accurately.
• Administer medications and treatments as prescribed by the physician and outlined in the care plan.
• Collaborate with the RN Case Manager and other team members to coordinate patient care effectively.
• Provide patient and family education to promote understanding of care and encourage independence.
• Ensure compliance with all clinical standards, safety protocols, and regulatory guidelines.
