Requirements
- Active LPN license in Pennsylvania
- A passion for home health and patient-centered care
- Strong communication and time management skills
- The ability to work independently—with full team support behind you
Responsibilities
- Deliver nursing care as outlined in each patient’s personalized care plan
- Partner with RNs, therapists, and other clinicians to ensure top-quality care
- Monitor and report patient conditions and changes with accuracy
- Build meaningful relationships with patients and their families
- Provide comfort, dignity, and hope with every visit
