Requirements

• Active LPN/LVN license (state required)
• Graduate of an accredited nursing program
• 1+ year of clinical experience
• Ability and willingness to travel (~50%)
• Reliable transportation, valid driver’s license, and auto insurance
• Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs

Responsibilities

• Deliver skilled nursing care under the direction of an RN or physician
• Follow and implement individualized Plan of Care for each patient
• Monitor patient conditions and report changes to the care team
• Educate patients and families on self-care and safety
• Administer medications and maintain accurate documentation
• Help prevent avoidable hospitalizations through proactive care
• Collaborate closely with RNs and interdisciplinary teams
