About The Position

The Licensed Practical Nurse (LPN) plays a crucial role in providing nursing care under the supervision of a registered nurse. This position focuses on patient assessment, treatment execution, and education, ensuring that patients receive comprehensive care in alignment with the organization's values of integrity, patient-centeredness, respect, accountability, and compassion.

Requirements

• Graduated from an approved Practical Nurses Education Program.
• Minimum one year experience in the acute care setting; home care experience preferred but not required.
• Licensed Practical Nurse with a valid Connecticut license required.
• Excellent verbal and written skills.
• Excellent clinical knowledge.
• Strong computer skills.

Nice To Haves

• Home care experience preferred.

Responsibilities

• Assesses patients within scope of practice at every home visit.
• Carries out treatments per MD orders and reports to RN case manager and/or clinical manager.
• Reports all changes in condition to MD.
• Provides patient/caregiver education as per care plan.
• Completes all documentation within 24 hours, including changes in condition and follow-up with providers or other disciplines.
• Attends all staff meetings, inservices, and other required meetings.
• Complies with all mandatory requirements, including Healthstreams.
• Follows policies and procedures.
