Requirements

  • Graduate of an accredited nursing program (Certificate, Diploma, or Associate Degree)
  • Active State LPN/LVN license
  • At least 1 year of clinical nursing experience
  • Ability to travel locally (approximately 50%)
  • Valid driver’s license, reliable transportation, and insurance
  • Ability to meet physical demands of the role, including lifting up to 50–100 lbs

Nice To Haves

  • Hospice or home health experience preferred

Responsibilities

  • Deliver skilled nursing care in accordance with the patient’s Plan of Care, physician orders, and established standards
  • Support patient goals through high-quality, individualized nursing visits
  • Communicate changes in patient condition to the RN Case Manager, physician, and care team
  • Promote patient safety and report unsafe conditions promptly
  • Educate patients and caregivers on self-care techniques
  • Assist with medication management and accurate documentation
  • Collaborate closely with the RN Case Manager for guidance and support
  • Help prevent avoidable hospitalizations and emergency room visits
  • Treat patients, families, and caregivers with respect, compassion, and professionalism
