Requirements

  • Graduate of an accredited Practical Nursing program (Certificate, Diploma, or Associate Degree)
  • Active state license as a Licensed Practical Nurse (LPN)
  • Minimum of 1 year of clinical nursing experience
  • Ability to travel up to 50% within the assigned territory
  • Ability to sit, stand, bend, lift, and move intermittently; must be able to lift up to 50–100 lbs
  • Valid driver’s license, current auto insurance, and reliable transportation

  • Home health experience preferred

Responsibilities

  • Provide skilled nursing care in the home under the direction of an RN and in accordance with the physician’s Plan of Care
  • Review and follow the Plan of Care prior to each visit to ensure safe, effective treatment
  • Document patient assessments, care provided, and progress accurately and timely
  • Communicate changes in patient condition, safety concerns, or environmental risks to the RN Case Manager and care team
  • Assist patients and caregivers with education on medications, treatments, and self-care techniques
  • Support patient safety using family and community resources
  • Collaborate closely with RN Case Managers during normal business hours for guidance and clinical support
  • Maintain accurate medication documentation and update medication profiles per agency guidelines
  • Deliver compassionate, respectful care to patients, families, and caregivers
  • Take proactive steps to help prevent avoidable hospitalizations and emergency room visits
