About The Position

The Home Health Intake Coordinator at St. Joseph Villa Homecare and Hospice is responsible for managing referrals throughout the intake process, ensuring compliance with state and federal regulations, and facilitating communication between patients and staff. This role is essential for maintaining the continuity of care and processing necessary documentation efficiently.

Requirements

  • Home Health Intake experience is preferred, but not required.
  • Proficiency in Microsoft Office Suite is a plus.
  • 1 year of experience as an intake coordinator is preferred.
  • Education: Associate degree is preferred.

Responsibilities

  • Manages referrals throughout the entire intake process, including retrieving referrals from the fax or email server and expediting the processing of medical, social, and financial information in the electronic medical record.
  • Ensures that all OASIS and plan of care documents are sent to the appropriate physicians, tracks the receipt of forms, and ensures that forms are returned and processed in compliance with regulations.
  • Works cooperatively with the clinical department to provide continuity of information for patients and staff.
  • Scans admissions documentation into the electronic medical record.
  • Processes financial and demographic information through appropriate Medicare and/or Medicaid systems to determine eligibility.
