We are currently seeking a Full-Time Intake Coordinator to play a vital role in ensuring seamless coordination and efficient management of patient care referrals. The Intake Coordinator is responsible for capturing and organizing all relevant referral information — from insurance verification to payment processing — ensuring accurate and timely setup of care services. You will coordinate multi-disciplinary visits, optimize the patient experience, and facilitate effective communication among healthcare professionals. Requirements
- High school diploma or GED required
- Experience with electronic charting required
- 1–3 years of Medicare/insurance billing, authorizations, or medical records experience
- Strong organizational skills and attention to detail
- Ability to prioritize multiple tasks while meeting deadlines
- Excellent written and verbal communication skills
- Adaptability in a dynamic healthcare environment
- Prolonged periods of sitting and working at a computer
- Ability to lift up to 15 pounds at times
Nice To Haves
- Familiarity with EHR systems and scheduling/billing software preferred
Responsibilities
- Receive referral requests from providers or pharmacies via phone, fax, or online portal
- Utilize multiple software systems to determine member plans and requirements
- Gather required documentation, including benefit eligibility verification
- Build cases in the utilization management system
- Evaluate cases requiring additional notifications to members, providers, and/or pharmacists
- Contact providers/pharmacies as needed to obtain additional information
- Ensure accurate data entry to prevent compliance or downstream issues
- Provide care updates to appropriate parties
- Prepare accurate, timely, and organized documentation
- Coordinate care with clients as needed
- Provide healthcare instructions to patients/families when appropriate
