About The Position

The Home Health Intake Clinician receives and processes admissions for patients requiring home care, hospice care or post-acute care services. This position works directly with patients and their families to determine their needs, medical history, physical and mental state, and special requirements, using this information to define what services are needed.

Requirements

  • Associate's Degree or Diploma from Accredited Licensed Practical Nurse Program - Required
  • Licensed Practical Nurse (LPN) State Licensing Board- Required
  • 2 Years Home Health Care Experience- Preferred
  • Driver's License- Required
  • Auto Insurance- Required

Responsibilities

  • Assist billing and tracking non-paid claims, including researching information and documentation for these other insurance-related billing problems.
  • Investigates and determines initial patient benefits coverage for requested services, which includes Medicare, Medicaid, private insurance, HMO's, PPO's, grants, special contracts, charity, and fee structures.
  • Assists patients, families, physicians, and clinicians via telephone with clinical issues, problems, and questions.
  • Works collaboratively with referral sources including hospital, ECF and ACO discharge planners and care coordinators to facilitate continuity of care and appropriate care.
  • Acts as a liaison with pharmacy, medical supplies, DME and other vendors providing care to VNS patients.
  • Accepts and processes physician orders via telephone.
  • Communicates with all referral sources to gain the necessary information to facilitate continuity of care and appropriate care of patients and clients including demographics, diagnoses, surgeries, hospital/ECF dates, physician's orders, and reimbursement or insurance information.
  • Completes appropriate referral forms and enters information into the computer.
  • Develops and implements clinical review systems for all pay sources to include admissions, continued stay, and concurrent and retrospective reviews.
  • Documents interventions taken and orders received as indicated.
  • Obtains managed care and insurance authorizations to determine the applicable coordination of benefits.
  • Schedules patient visits for field staff to ensure that the frequency of visits are compliant with the plan of care.
