About The Position

Pentec Health is seeking a dedicated and skilled Home Health Infusion Registered Nurse to join our team in Rockford, IL. This full-time, permanent position offers the unique opportunity to create your own schedule to meet the needs of patients while traveling within the area, potentially covering distances of up to 200 miles. In this role, you will be responsible for providing comprehensive nursing care to patients with implanted intrathecal drug delivery devices. This includes conducting thorough patient assessments, performing nursing interventions, managing pump refills, making dose adjustments, and providing essential patient education. As a Home Health Infusion Registered Nurse, your work will not be limited to home visits; you may also provide care in hospitals, clinics, or other healthcare settings. This non-remote role requires a commitment to delivering high-quality nursing care that is tailored to each patient's unique needs. You will be expected to adapt nursing care based on mutually agreed-upon goals, following established policies, procedures, and standards of care guided by the nursing process and shared governance model. You will also participate in the ANCC accredited Nurse Fellowship Program upon hire, which will enhance your skills and knowledge in this specialized area of nursing. Your responsibilities will include educating patients and caregivers on healthy lifestyle choices, promoting informed decision-making, and utilizing integrative care techniques to assist patients with symptom management, ultimately improving their quality of life. Accurate and timely documentation is crucial, as is effective communication with members of the multidisciplinary team. You will be responsible for all elements of the nursing process, including assessment, nursing diagnosis, outcomes planning, implementation, and evaluation for each patient. Collaboration with prescribing physicians and caregivers is essential to ensure comprehensive patient care.
