About The Position

The primary function of the Home Health Infusion Nurse is to provide exceptional in-home IV infusion therapy, nursing care, and response management. In addition, the Home Health Infusion Nurse will administer and monitor patients in the home during treatment and provide the necessary education on treatment regimen, condition management and treatment self-administration when appropriate. Requires the ability to work independently, be detail oriented and to handle multiple tasks in a fast paced environment

Requirements

  • Graduate of an accredited school of nursing
  • Bachelor of Science Nursing or equivalent similar health related field
  • 2 years recent nursing experience. Preferably in Skilled Homecare, Emergency Room, Labor and Delivery, Infusion Center, Oncology or Med/Surg
  • Knowledge of computer applications in order to perform job duties: EMR, Outlook, etc.
  • MN Registered Nurse (RN) License
  • Basic Life Support (American Heart Assoc or Red Cross)
  • Drivers License
  • Current and adequate automobile insurance policy
  • Oncology and chemotherapy administration experience required.

Nice To Haves

  • 1 year previous home care/home infusion therapy experience
  • Experience with pediatrics, geriatrics and oncology patients
  • Certification in infusion therapy, oncology or critical care

Responsibilities

  • Deliver care as delegated by physician, providing accurate and clinically appropriate infusion therapy assessments and interventions for all therapies, access devices and disease processes; assessing patient for health status changes, support systems and home environment, updating the care team and documenting in clinical record as indicated.
  • Partner with the patient, family and care team in developing and implementing plan of care with specific goals under the direction of the physician. Initiate and coordinate care conferences with the participation of all members of the care team as needed.
  • Provide education and support to patients and caregivers that enable them to safely and effectively self-administer medications and solutions. This includes storage, aseptic technique, equipment use, side effects and potential complications.
  • Coordinate the discharge of patients when goals are met with the physician and other disciplines to achieve an orderly transition for the patient/family.
  • Provide patient clinical care and coordination in a variety of environments including patient's home, infusion suites and hospital.
  • Train patients in the self-administration of treatments and how to quickly recognize and respond to symptoms of adverse physical reactions to treatments.
  • Maintain open communication with Charge Nurse, Nursing Leadership, Staffing Coordinator and other team members regarding scheduling, patient assignments and clinical matters
  • Coordinate with Pharmacy team, and/or Nursing Leaders to address changes in patient condition or drug management issues.
  • Document all interactions and communication with patients, plan of treatment, assessment, treatments, patient responses/reactions to treatment, status of disease process, and training activities in a timely manner using company issued tools
