About The Position

The Home Health & Hospice Social Worker LMSW / LCSW at Masonicare is responsible for delivering high-quality Medical Social Work services to patients and their families, ensuring that care reflects the agency's philosophy and standards. This role involves comprehensive psychosocial assessments, effective communication with interdisciplinary teams, and the development and evaluation of care plans to address the psychosocial needs of patients across various age groups.

Requirements

• Master's degree in Social Work and/or Board Eligible.
• Two years of experience in social work in a health care setting.
• Current CT State LMSW Licensure (Licensed Masters of Social Work).
• Maintains a current Driver's License and auto insurance.
• Possesses reliable transportation.

Responsibilities

• Provides care and counseling to sick palliative care and hospice patients and families.
• Communicates effectively with members of the IDT to promote a team-centered approach to care.
• Attends Hospice IDT meetings and other relevant care meetings as applicable.
• Performs comprehensive psychosocial assessments addressing the relationship between medical conditions and psychosocial functioning.
• Meets social work goals related to patients' medically related psychosocial problems through effective problem resolution.
• Provides education and counseling to patients and their families regarding care needs.
• Develops, implements, and evaluates the plan of care, making necessary revisions as needed.
• Makes recommendations for additional home health care and community services in a timely manner.
• Coordinates the plan of care and maintains continuity of client care with other health professionals.
• Documents patient visits accurately and timely using appropriate documentation tools.
• Communicates effectively and tactfully with clients, considering their age, cultural diversity, and physical condition.
• Reports to work on time and completes annual education and medical requirements.
• Participates in the agency's Quality Improvement program as required.
