About The Position
Solid Care Home Health & Neocare Palliative Hospice, Inc. is an agency in Woodland Hills, CA that specializes in palliative care, generally from birth to advanced age. We're family-centric rather than patient-centric. Our team works closely with the family and the physician to deliver seamless physical, psychosocial, and spiritual care. We are professional, agile and supportive. We're seeking a Registered Nurse Case Manager to join our hospice and home health team. Primarily responsible for providing professional nursing care with input from the IDT (interdisciplinary team), patient and family, assesses, develops, implements and evaluates the plan of care to meet the needs of individuals and families within their home environments. Specializing care to young, middle and older adults. This position would cover the following areas Ontario, Riverside and surroundings.
Requirements
- Must have current California Registered Nurse License
- 1 year of recent nursing experience in home health, hospice, palliative care, acute care &/or skilled nursing
- Current CPR/BLS
- Current CA driver’s license with proof of insurance.
Responsibilities
- Handle a caseload of assigned patients.
- Conduct home visits for patients (Travel required for patient visits).
- Performs admission assessment and Start of Care's
- Develops written plan of care in coordination with an interdisciplinary Team (IDT).
- Assess patient needs and provide appropriate nursing services.
- Oversees implementation of plan, evaluates and changes plan as necessary to meet patient/family needs.
- Communicates all changes to IDT and primary MD.
- Maintains timely patient documentation (Charting)
- Complete reports and patient assessments
- Implements and maintains established hospice policies and procedures as well as Standards of Care and regulatory requirements.
