About The Position

The Home Health and Hospice RN – Case Manager is responsible for the overall care coordination of each home health or hospice patient cared for by the agency from the time of referral to the time of discharge. Ensures the timely delivery of appropriate service, which promotes quality and cost-effective health care outcomes, working within established agency policies and Home Health and Hospice regulations. Works closely with visiting nursing staff, providing direct patient care to ensure that care is goal-directed and patients' needs are being met. The HHH RN – Case Manager acts as a patient advocate and primary contact for patients, caregivers, family, insurance companies, doctors, and staff to ensure effective communication regarding patient care.

Requirements

Associate’s Degree (A.D.), Diploma, Bachelor’s Degree (B.S.), or Master of Science (M.S). from an approved school of nursing.
Minimum of two (2) years’ previous home health or hospice experience or in combination with supervisory experience.
Current State of Ohio Registered Nurse licensure.
BLS certification or obtained within 30 days of hire.
Current valid Ohio Driver’s license.
