The Home Health & Hospice Data Analyst will serve as a key strategic resource supporting the data needs of a healthcare venture focused on delivering high-quality, cost-effective post-acute care. This role will analyze and interpret clinical, operational, financial, and referral data to support performance improvement, business development, and compliance initiatives for both Home Health and Hospice lines of service. The analyst will work collaboratively across leadership and post-acute teams to provide actionable insights and enhance data-driven decision-making.

  • Build, maintain, and automate dashboards and reports that monitor KPIs across clinical quality, financial performance, referrals, and operations.
  • Track and analyze metrics such as admissions, referral, Intake process, productivity, readmission rates, length of stay, Payor trends, and other areas of opportunity.
  • Provide insight into referral patterns, physician behavior, and market trends to support business development strategies.
  • Assist leadership with forecasting, budgeting, and pro forma modeling for program expansion or new initiatives.
  • Provide analytics for performance improvement initiatives and value-based care readiness.
  • Develop shared scorecards for hospital and post-acute leadership.
  • Support partnership governance by preparing quarterly data summaries, trend reports, and leadership reporting.
  • Ensure data accuracy and alignment between EMR systems (e.g., Homecare Homebase, EPIC) systems.
  • Partner with IT and EMR teams to validate data sources, definitions, and workflows.
  • Bachelor’s degree
  • Minimum 3 years of experience in healthcare analytics, preferably in post-acute or hospital settings
  • Experience with Home Health and/or Hospice data sets and workflows preferred
  • Proficiency in Excel, Power BI, Tableau, or other data visualization platforms
  • Familiarity with EMR systems such as Homecare Homebase, EPIC, Trella
  • Ability to analyze complex data and present insights clearly to non-technical audiences
  • Excellent communication, time management, and cross-functional collaboration skills
  • Experience working with healthcare partnerships or integrated health systems
  • Working knowledge in home health and hospice industry
