Requirements
- Current RN License
- Valid Driver’s License
- CPR Certification
- Valid Auto Insurance
- At least 2 years of Home Health RN experience preferred
- HomeCare HomeBase experience preferred
Responsibilities
- Complete skilled comprehensive assessments in patient homes
- Develop and adjust individualized plans of care in real time
- Recognize changes early and coordinate with physicians immediately
- Educate patients and families so they can stay safe at home
- Supervise and support HHA/LVN staff in the field
- Ensure compliance, documentation accuracy, and payer requirements
