About The Position

We are seeking PRN Registered Nurses for Home Health coverage in the Prairie View / Hempstead area and surrounding communities. 📌 PRN requirement: Minimum 15 visits per week ---------------------------------------- 💙 WHY NURSES CHOOSE THIS ROLE ✔ Flexible PRN schedule with steady visit expectations ✔ Work independently in the field (no hospital overwhelm) ✔ One-on-one patient care in the home setting ✔ Strong clinical support team when needed ✔ Meaningful time with patients—not rushed care ---------------------------------------- 🩺 WHAT YOU’LL DO As a Home Health RN, you will: Perform comprehensive patient assessments (physical, psychosocial, environmental) Develop and update individualized plans of care Provide skilled nursing services (wound care, disease management, medication education, etc.) Educate patients and families on self-care, safety, and chronic disease management Coordinate care with physicians and interdisciplinary team members Supervise Home Health Aides and LPN/LVNs as needed Document care accurately and support payor compliance requirements ----------------------------------------
Requirements
- Active Texas RN license
- Ability to consistently complete minimum 15 visits per week (PRN requirement)
- Strong independent clinical judgment
- Comfortable working in home environments
- Reliable transportation
- Current RN License
- Valid Driver’s License
- CPR Certification
- Valid Auto Insurance
Nice To Haves
- Home Health or acute care experience preferred
- At least 2 years of Home Health RN experience preferred
- HomeCare HomeBase experience preferred
Responsibilities
