About The Position

Certified Occupational Therapy Assistant (COTA) South Bend Home Health-paid mileage Home Health – Join Our Mission at Trinity Health Are you passionate about helping others live their best lives at home? We’re looking for a Certified Occupational Therapy Assistant (COTA) to join our compassionate home care team. In this role, you’ll partner closely with an Occupational Therapist to deliver thoughtful, hands-on care that supports client independence, safety, and well‑being. What You’ll Do As a COTA with us, you will: Provide direct occupational therapy treatments in clients’ homes based on an established care plan Support clients in using adaptive equipment and therapeutic devices Teach families how to assist safely with home exercises and daily activities Observe and report changes in a client’s condition to the OT and care team Help ensure the client’s home environment supports their goals and safety Document visits accurately and submit notes in a timely manner Communicate regularly with the OT, care team, and scheduling staff Participate in quality improvement and team meetings as needed Every day, you’ll make a meaningful difference in someone's life. What You Bring Current state licensure/certification as an Occupational Therapy Assistant Graduate of an AOTA‑approved OTA program (or equivalent experience/credentials) At least 6 months of experience working under a licensed OT Home health experience preferred, but not required Valid driver’s license and reliable transportation Ability to work independently, stay organized, and communicate professionally Commitment to our mission, values, and high‑quality client care Physical Requirements You’re comfortable with: Frequent standing, walking, lifting (up to 35 lbs) Occasional bending, stooping, or repositioning of clients Working in a variety of home environments and adapting as needed Why Join Trinity Health? Requirements

  • Current state licensure/certification as an Occupational Therapy Assistant
