About The Position

Coordinates home health referrals, intake, and scheduling to ensure timely start of care. Helps plan and maintain staffing patterns for patient care, meeting continuity of care and acuity level of care needs. Works with multidisciplinary team to ensure continuity in patient care and to promote positive patient outcomes. Takes lead role and is knowledgeable in OASIS assessment from admission to discharge. Takes lead role with compliance activities such as monitoring timely Start of Care (SOC), OASIS requirements, and visit documentation Assists with work area in PI planning and development. Demonstrates knowledge in Home Health diagnosis coding. Understanding of basic operational financial concepts, including budgeting, cost awareness, and resource utilization. Serves as a clinical resource person and assists nursing personnel in assessing patient/family status to plan nursing care. Communicates with family and other health care professionals regarding care and discharge planning.

Requirements

  • Strong communication, organization, and time‑management skills
  • Ability to manage multiple priorities in a fast‑paced environment
  • Customer‑service focused with a commitment to patient‑centered care
  • Graduate of an accredited school of nursing. BSN preferred.
  • License to practice professional nursing in the state of Ohio.
  • Minimum 1–2 years of experience in Home Health preferred.

Nice To Haves

  • Certification in coding and OASIS preferred.
