About The Position

Summary: Responsible for providing personal care according to written Care Plan prepared by the Registered Nurse under the supervision of the RN and order by the physician.

Requirements

  • Current CNA certification
  • Prior experience as a C.N.A.
  • Satisfactory completion of the Home Health Aide Competency Examination
  • Ability to provide own transportation
  • Ability to carry out directions
  • Must demonstrate basic knowledge of all procedures included in the Home Health Aide job description
  • Must be able to assess safety of home situations for self, patients, and other staff members, including physical and psychological dangers
  • Must have the ability to effectively cope with patients, families, and all others with varying backgrounds

Nice To Haves

  • Prior hospital experience, and/or equivalent experience in nursing home or public health nursing, is preferred
  • Previous Home Health Aide experience preferrable

Responsibilities

  • Follows the written instructions on the Aide Assignment Sheet for patient care, as prepared by the Registered Nurse
  • Assists patients in activities of daily living (ADL): Personal hygiene, bathing, shampoo, shaving, mouth care, skin care, eating, dressing, elimination
  • Assists patient with ambulation, transfers, and exercise
  • Performs household services essential to health care at home, including personal laundry, making bed as well as other small household tasks
  • Cares for patient’s surroundings as necessary to maintain, as much as possible, a safe environment
  • Maintains cheerful and positive attitude while providing personal care and supportive services as assigned by the Registered Nurse
  • Engages in conversation that is neither personal nor controversial in nature
  • Reports to the Registered Nurses/Nursing Supervisor changes in the patient’s condition and/or emotional status and needs
  • Completes appropriate documentation according to Hospice policies utilizing the medical record software system via the assigned agency device; Initiates documentation in the patient’s home on the device and completes after the visit.
  • Maintains patient confidentiality while using EMR device
  • Attends all mandatory in-service programs and staff meetings
