About The Position

The Home Health CNA position in the Racine area involves making home visits to specified patients and providing personal care according to the individual plan of care developed by the registered nurse. The role includes performing routine personal care activities such as bathing, dressing, and skin care, as well as assisting patients with mobility and meal preparation. The CNA is responsible for documenting patient health information and communicating with healthcare team members. The position requires a commitment to providing care appropriate to the age of the patients served and participating in quality improvement activities.

Requirements

  • Nurse Aide in good standing with the Wisconsin Nurse Aide Registry.
  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) within 6 months.
  • A valid driver's license issued by the Division of Motor Vehicles.
  • Advanced training beyond High School in an accredited or approved program in Nursing Assistant.
  • Typically requires 1 year of experience in providing patient care within a home health care setting.

Nice To Haves

  • Experience working with elderly, convalescent, and/or disabled persons.
  • Excellent communication and listening skills.
  • Ability to prioritize tasks for efficient use of visit time.

Responsibilities

  • Makes home visits to specified patients and provides personal care according to the individual plan of care developed by the registered nurse.
  • Performs routine personal care activities for patients, including bathing, dressing, skin care, and simple wound care.
  • Obtains patient's temperature, pulse, and respiratory rate; documents and updates patient's care and health information.
  • Assists patients with transfers in and out of bed, chair, bathroom, and/or wheelchair.
  • Assists patients with planning and preparing meals, feeding, and testing urine for sugar and acetone.
  • Performs non-routine personal care activities requiring additional instruction by a registered nurse.
  • Provides home management activities necessary to maintain and facilitate patient's health.
  • Communicates with physicians and other members of the healthcare team.
  • Acts in a prompt, friendly, and professional manner when dealing with patients and staff.
  • Participates in chart audits and continuous quality improvement activities.
