Requirements

• Active RN or LPN license required
• Home Health experience required
• Strong knowledge of home health regulations and compliance standards
• Attention to detail and ability to work independently
• Strong communication and problem solving skills
• Comfort working in a fast paced, deadline driven environment

Nice To Haves

• Experience with clinical documentation review or QA preferred
• Experience with EMR systems and OASIS a plus

Responsibilities

• Review clinical documentation for accuracy, completeness, and compliance
• Ensure documentation meets state, federal, and payer requirements
• Audit charts and identify areas for improvement or correction
• Collaborate with clinicians to resolve documentation issues and provide guidance
• Support quality assurance and performance improvement initiatives
• Assist with OASIS review and documentation standards as applicable
• Maintain organized records and meet deadlines for chart review and submission
• Communicate effectively with clinical and administrative teams
