About The Position

The Home Health Clinical Manager has direct responsibility for patient care services provided by the Home Health program. Acts as alternate Nursing service line leadership. This position serves as a pivotal point in communication with field staff, support staff, referral sources, other managers/departments to ensure extraordinary service delivery, and in close coordination with the Home Health Administrative Manager. The Clinical Manager provides orientation, training and support for both newly hired and existing clinicians, serves as a mentor for case management, and participates in the assessment and planning of appropriate care.

Requirements

  • Two years of experience in Homecare
  • Current Montana RN license
  • ASN, bachelor’s degree or better directly related to licensure
  • One year of supervisory experience is required
  • Excellent analytical and interpersonal skills required

Responsibilities

  • Ensures the delivery of home health services in compliance with program/payer rules and physician orders.
  • Reviews, plans, coordinates, manages and evaluates the activities of a multidisciplinary team to ensure the delivery of high-quality patient care services.
  • Utilizes evidence-based practice in the field and implements standards of practice that guide practice improvement initiatives. Will spend a minimum of 50% of the time in direct observation of clinical staff providing patient care.
  • Maintains a consistent focus on Key Performance Behaviors (KPBs) to drive favorable clinical and functional outcomes.
  • Provides clear expectation of performance and measures adherence of best practice.
  • Gives staff the tools/education and support to meet the best practice.
  • Assures current and accurate clinical records in compliance with documentation requirements and program specific rules for coverage, qualifying criteria, and Medicare Conditions of Participation. Performs chart audits to identify areas that require additional teaching, training and clinician performance.
  • Coaches and mentors staff, and assists in onboarding and orientation.
  • Assist clinical staff in problem solving related to routine service delivery, crisis, or other critical situations.
  • Promotes safe work practices and work environments to protect the safety and wellbeing of employees and patients
  • Serves on the agency’s QAPI/Compliance Committee. Provides the necessary follow-up training with staff to meet KPB’s. Implements care delivery best practices into the field consistent with outcomes of QAPI initiatives. Monitors care delivery in the field for competence and compliance.
  • Coordinate the best experience for students using on-site and field internships and shadows with the goal of positioning the PHC as the employer of choice for services we provide.
  • Participates in clinical management on-call rotation.
  • Available to all clinicians for questions and support when needed.
  • Performs other duties as assigned by supervisor.
