Job Description: Home Health Certified Occupational Therapist Assistant Full-Time | Field-Based Genesee County, MI At Elara Caring, we believe the best care happens where patients feel most comfortable—at home. Requirements

  • Graduate of an accredited occupational therapy assistant program
  • Active, unrestricted COTA certification in the state of practice
  • Minimum 1 year of clinical experience as a COTA
  • Comfortable working independently in a field-based role
  • Ability and willingness to travel within the assigned coverage area (approximately 50%)
  • Reliable vehicle, valid driver’s license, and current auto insurance
  • Ability to lift up to 50–100 lbs and perform physical tasks associated with patient care

Responsibilities

  • Provide occupational therapy services to patients in their homes under the supervision of an Occupational Therapist
  • Follow and implement individualized plans of care to support patient independence and safety
  • Monitor patient progress and communicate changes in condition to the OT, physician, and care team
  • Educate patients and caregivers on therapy goals and home exercise programs
  • Maintain timely, accurate clinical documentation
  • Collaborate closely with interdisciplinary team members to ensure high-quality care
  • Help reduce avoidable hospitalizations and ER visits through proactive, patient-centered care
