About The Position

The Home Health Case Manager is responsible for coordinating and managing patient care within a home health agency. This office-based role requires a qualified individual to oversee field staff, assess patient needs for discharge planning, and ensure effective communication with healthcare providers. The ideal candidate will have experience in home health and case management, with a focus on delivering quality care to patients and their families.

Requirements

• Must be a Licensed Vocational Nurse (LVN) or Registered Nurse (RN).
• 1 year experience in Home Health.
• Knowledge of case management and managed care processes.
• Proficient in Microsoft Office and WellSky (Kinnser) software.
• Strong organizational, problem-solving, and communication skills.

Responsibilities

• Coordinate field staff.
• Assess inpatients to determine discharge planning needs.
• Consult with physicians to obtain essential information including the post-discharge treatment plan and communicate this treatment plan to other healthcare providers.
• Document assessments, discharge plans, services, and outcomes in the medical record.
• Make referrals and arrange for needed services for home care including physicians, hospitals, IPA's, and HMO's.
• Meet the needs of patients and their significant others by ensuring the provision of clinical social services.
• Maintain communication and documentation in EMR (Kinnser).
• Responsible for QA of notes submitted by clinicians, including QA/Audit/Review of medical records.
