Requirements

Are a licensed RN
Have 3+ years of home health experience and at least 1 year in a leadership role
Can navigate OASIS, EMR platforms, and documentation like a pro
Love leading teams and making people feel valued
Are organized, compassionate, and cool under pressure

Responsibilities

Lead and support a team of skilled clinicians to ensure quality, compliance, and continuity of care.
Oversee coordination of patient services, care planning, and interdisciplinary communication. Collaborate with physicians, patients, and families to make care truly personal.
Ensure field staff receive the education, resources, and real-time support needed to deliver outstanding care.
