About The Position

Requirements

  • Active RN license in Maryland.
  • Minimum of 3 years of clinical experience in home health or a related healthcare setting.
  • Strong organizational, communication, and interpersonal skills.
  • A passion for supporting staff and fostering a collaborative team culture.

  • Experience in case management or care coordination preferred.

Responsibilities

  • Oversee and coordinate patient care for the Silver Spring office.
  • Provide support, guidance, and communication to clinical staff to ensure high-quality care delivery.
  • Collaborate with interdisciplinary teams to create individualized care plans that promote patient health, independence, and dignity.
  • Monitor and evaluate patient outcomes, implementing follow-up or care adjustments as needed.
  • Ensure compliance with all regulatory and accreditation standards.
  • Support staff through ongoing education, training, and professional development.
  • Build strong relationships with patients, families, and community partners.
