Requirements

Excellent communication & relationship-building skills
Strong organization and time-management abilities
Knowledge of Medicaid, Medicare, and Indiana home care guidelines
Background in case management, social work, or healthcare
Tech-savvy with EHR or case management systems

Responsibilities

Client Assessments & Care Plans – Evaluate needs and design personalized care plans
