About The Position

The Junior Marketer / Intake Coordinator supports the growth of the agency by managing incoming referrals, ensuring a smooth intake process for patients and families, and assisting with basic marketing and referral tracking activities. This role works closely with clinical directors, administrator, central intake team, and field staff to provide an efficient, positive referral experience for all stakeholders. Schedule: Full-Time, Weekdays, 8:30 AM - 5:00 PM

Requirements

  • High school diploma or equivalent required; some college or healthcare-related coursework preferred.
  • 1–2 years of experience in healthcare, home health, hospice, or a medical office setting preferred.
  • Experience with EMR systems, referral portals, and fax/email document management preferred.
  • Strong customer service skills with the ability to communicate professionally with patients, families, facilities, and internal staff.
  • Excellent organization, attention to detail, and multi-tasking abilities in a fast-paced environment.
  • Demonstrated ability to handle confidential and sensitive information in compliance with HIPAA and company policy.
  • Regularly required to walk, sit, stand, bend, reach, talk, hear, kneel, crouch, stretch, twist, and move about.
  • Required vision includes close, vision, distance, peripheral, and the ability to adjust focus.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Ability to carefully consider all information and make impacting decisions on a regular basis.
  • Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager.
  • Job requires accepting criticism and dealing calmly and effectively with high-stress situations.
  • Ability to communicate effectively, both orally and in writing.

Responsibilities

  • Coordinate incoming home health and hospice referrals from hospitals, facilities, and community partners.
  • Gather required clinical and demographic documents from referral sources to complete the intake packet.
  • Prepare referral packets for the DON and other clinical leadership for timely review.
  • Verify insurance coverage and benefits for all new referrals, escalating issues as needed.
  • Input new referrals into the EMR accurately and in a timely manner.
  • Work directly with the central intake team to ensure complete and compliant referral information.
  • Communicate with facilities, staff, and patients (and/or families) regarding the referral and admission process, providing clear expectations and updates.
  • Answer incoming phone calls regarding referrals, routing calls and messages appropriately.
  • Monitor faxes and promptly distribute faxed referrals, orders, and other incoming documents to the appropriate team members.
  • Monitor hospital and referral portals for incoming referrals and updates, ensuring they are captured and processed quickly.
  • Compile data and referral reports to track referral volume, referral sources, and conversion to admissions.
  • Assist with basic marketing tasks, such as maintaining referral source contact lists, supporting follow-up communication to key referral partners, and providing data needed for marketing/outreach planning
