About The Position

Requirements

• Must be 18 years or older and have completed a state-approved Home Health Aide or Nursing Assistant training program.
• Registered on the Wisconsin registry if working in Wisconsin.
• Previous work experience as a certified nursing assistant within the past 12 months is preferred.
• Experience with end-of-life or geriatric populations is preferred.
• Clear written and verbal communication skills and excellent interpersonal skills are required.
• Ability to work independently and follow directions accurately.
• Basic computer and technological device skills are necessary.
• Valid driver's license, proof of auto insurance, and acceptable driving record are required.
• Current Basic Life Support certification for Health Care Providers from an approved program is required.

• Experience with end-of-life care or geriatric populations is preferred.

Responsibilities

• Provide personal care and homemaking services to patients in their residence.
• Follow individualized patient care plans under the supervision of a Registered Nurse or therapist.
• Maintain open communication with the interdisciplinary team to foster a team atmosphere.
• Recognize and report problems or concerns to appropriate personnel.
• Record and maintain accurate patient data and documentation.
• Drive to patients' residences in all weather conditions.
