About The Position

UPMC Home Healthcare and Family Hospice is seeking a compassionate and dependable Home Health Aide to provide exceptional, one-on-one care to home health and hospice patients throughout Erie County. This field-based role offers the opportunity to make a meaningful impact by supporting patients in the comfort and dignity of their own homes. This position is a full-time, Monday-Friday position from 8:00AM-4:30PM with rotating weekend shifts. If you are passionate about making a difference in patients’ lives, we encourage you to apply!

Requirements

  • High School Diploma or GED - Required
  • Minimum of 75 hours of Nurse Aide training - Required
  • CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
  • Automotive Insurance
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Driver's License

Nice To Haves

  • One (1) year of experience - Preferred
  • Certified Nurse Aide - Preferred
  • If no previous nurse aide training, applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire.

Responsibilities

  • Providing safe, direct personal care under the supervision of licensed professionals
  • Monitoring patients’ conditions and promptly reporting changes to the interdisciplinary care team
  • Supporting patient comfort, well-being, and overall satisfaction
  • Completing accurate and timely documentation in accordance with agency and regulatory standards
  • Collaborating effectively with team members to ensure coordinated, high-quality care
  • Participating in required meetings, training, and annual competency evaluations
  • Adhering to organizational policies, standards of conduct, and compliance requirements
