Optimal Care Home Health Aides are paraprofessional members of the home health interdisciplinary team who work under the supervision of a Registered Nurse and perform various services to meet patient personal needs and to promote comfort. The Home Health Aide is responsible for observing the patient, and reporting and documenting observations and care performed. A Home Health Aide works in a manner that promotes quality, continuity, and safety of patient care. In this role you will be responsible for:

Requirements

  • Must be 18 years of age or older
  • Completed a minimum of 75 hours of classroom and supervised practical training (Sixteen hours of classroom must be completed prior to a mandatory 16 hours of practical training)
  • Must hold current certification as a nurse aide and be in good standing on the state nurse aide registry
  • Ability to take verbal and written instruction well
  • Works in a team responsibility and independently without direct supervision
  • Records and reports observations and activities
  • Demonstrates good communication skills and ability to read, write, and verbally report
  • Demonstrates knowledge and skills necessary to provide care to and communicate with primarily the geriatric and adult population
  • Demonstrates knowledge of the principles of the physical, emotional, and developmental needs of the geriatric and adult population
  • Able to assess data reflecting the patient's status and interpret the appropriate information needed to identify each client's requirements relative to their age-specific needs
  • Demonstrates computer skills sufficient to use the electronic medical record effectively
  • Valid Driver's License
  • Reliable transportation and valid automobile insurance coverage

Nice To Haves

  • High school diploma or GED
  • 1-year experience as a Hospice Aide, Home Health Aide, or Certified Nursing Assistant

Responsibilities

  • Assists with: Personal hygiene
  • Ambulation and exercise
  • Reporting changes in the client's condition, including skin condition
  • Nutritional support including preparing meals, meal set up, and assistance with feeding and promoting oral intake
  • Light household activities
  • Reading and recording temperature, pulse, respiration, weight, and blood pressure
  • Applies safety principles and proper body mechanics while assisting patient with ambulation, exercise, client transfers, range of motion, position, and light household chores.
  • Prepares and provides adequate nutrition and fluid intake: provides medically prescribed diets under supervision and as instructed in the Aide plan of care.
  • Performs homemaking and other environmental services that facilitate receiving sanitary and safe care at home.
  • Assists patients with reminders for medications that are ordinarily self-administered.
  • Observes, reports, and documents changes in patient status or expectations service promptly to a supervisor or other professional.
  • Understands basic elements of body functioning and reports verbally and in writing changes in patient status and body functions as indicated.
  • Recognizes emergency situations and implements appropriate emergency plan and protocols when indicated.
  • Demonstrates good basic infection control and prevention practices
  • Accurately documents care provided and submits visit records as per agency guidelines.
  • Attends in-service programs according to agency policy. Attends patient care conferences as directed.
  • Renders services in strict accordance with the written plan of care.
