SUMMARY OF JOB:
Provides personal care and assistance to Home Care and Hospice clients under the supervision of the Case Manager and Clinical Manager.

Requirements

• High School diploma or equivalent preferred
• Must be able to read and understand written instructions
• Must be able to take direction via telephone in patient’s home
• Must be able to make meaningful observations and report changes in patient condition
• Must have basic knowledge of computer applications and ability to enter patient information into an electronic device.
• Successfully pass all competency exams.
• Qualified for employment under current state (Pennsylvania or New Jersey) state regulations, current driver’s license and access to a car, current car registration, and current car insurance.
