About The Position

Performs a variety of in-home services for home health participants to provide supportive services.

• High school diploma or GED preferred.

Responsibilities

• Assists with implementation of the domestic plan of care which may include but is not limited to; preparing meals, running errands, house cleaning and doing laundry.
• Maintains accurate documentation in accordance with policies and procedures.
• Communicates participant concerns or changes in participant status to supervisor.
• Home Health services may be provided in a variety of locations including but not limited to participant residence.
