JOB SUMMARY Under direction of registered nurse, licensed physical therapist, or a licensed occupational therapist provides basic home nursing services to patients.
Requirements
- High School Diploma or equivalent
- 1 year of full-time experience in direct client care in an institutional setting (hospital or nursing facility)
- 1 year of full-time experience within the last five (5) years in direct client care in an agency setting.
- (BLS) BASIC LIFE SUPPORT course accredited by the American Heart Association (AHA) or American Red Cross (ARC)
- (DRIVERS) DRIVERS LICENSE TX Driver's License and proof of driver's insurance
- (CNA) CERT NURSE ASSISTANT Nurse Aide certification listed on the HHSC nurse aide registry (NAR).
Responsibilities
- Performs duties as assigned on the home health care plan prepared by a registered nurse.
- Obtains and documents patient vital signs, including temperature, weight, respiration, and pulse.
- Reports changes in patient's mental or physical condition or in home situation to nursing supervisor.
- Assists other members of the nursing team and/or other disciplines.
- Provides personal care assistance to patients with all Activities of Daily Living (ADLs), light housekeeping, meal preparation, and other specialized duties as needed.
- Provides assistance to patients with ambulation, safe transfers, and exercises as assigned.
- Maintains patient records and charts to ensure an accurate record of treatment.
- Maintains confidentiality of patient data.
- Assists in maintaining a healthy, safe environment.
- Attends mandatory in-service educational programs as required by federal and state regulations.
- Performs other related duties as assigned or requested.
