Organization: VHCS Clinical - Private Duty Job Summary: The Home Health Aide is a Certified Nurse Assistant that provides direct personal and supportive care for home care patients on an intermittent or hourly basis, based on physician orders. The Home Health Aide provides patient care and follows tasks as assigned on the patient’s individual plan of care, designed by the Registered Nurse. The Home Health Aide reports directly to the Clinical Staff Lead. The Home Health Aide. This position works in conjunction with other members of nursing service, ancillary services, physicians, patients, and families. It collaborates with various members of the healthcare team to ensure quality of care across the continuum. Requires the ability to be self-directed and quality focused. Interpersonal communication skills are a necessary function of this position.

Requirements

  • Certified Nursing Assistant - Certification - Others
  • Relevant Work Experience
  • Less than 1 year
  • High School Diploma or GED

Responsibilities

  • Reviews and performs tasks as define on the personal care plan of the individual patient.
  • Responsibilities vary based on the care plan but may include direct assistance with activities of daily living, bathing, toileting, obtaining vital signs, intake and outputs, weights, transferring, provides facial shaving, oral care, hair care and dressing.
  • The Home Health Aide reports any abnormal findings to the nurse and/or CSL.
  • The Home Health Aide communicates with the clinical team in a timely manner.
  • The Home Health Aide works under the supervision of the Registered Nurse or Qualifying therapist.
  • The Home Health Aide adheres to practices and policies outlined to ensure patient safety and follows infection control standards.
