About the Team The Home Care and Hospice team provides compassionate, skilled care to patients in their homes, supporting comfort, dignity, and independence during recovery, illness, or end-of-life. This multidisciplinary team works closely to deliver individualized, patient-centered services. Mission for Home Care/Hospice Aide The Home Care/Hospice Aide provides compassionate, personalized care in the home setting by supporting patients’ daily living needs, safety, and comfort under the guidance of licensed healthcare professionals. Position Type This is a casual position working on an as-needed basis.

Requirements

  • Current, valid Minnesota driver’s license and proof of auto insurance.
  • Follows instructions accurately and perform care tasks with precision.
  • Communicates effectively both verbally and in writing with the healthcare team and clients.
  • Maintains accurate records and documentation.
  • Supports infection control and safety protocols consistently.
  • Demonstrates reliability and responsiveness in home care settings.
  • Ability to organize, coordinate and prioritize tasks.
  • Maintain therapeutic boundaries with patients.

Nice To Haves

  • Certified Nursing Assistant preferred.
  • Previous Home Care/Hospice experience preferred.

Responsibilities

  • Provide personal care, mobility support, meal preparation, and hygiene assistance based on individualized care plans.
  • Follow infection control and safety procedures, including universal precautions and environmental safety practices.
  • Support daily living and promote independence through exercises, positioning, and use of assistive devices as directed.
  • Report health observations and changes in condition to the Home Care team; maintain accurate documentation.
  • Participate in staff meetings and required training; communicate with the team regarding scheduling and care updates.
